LTC-Site Evaluation Presentation Summary

During the site evaluations for this rotation, I always started by getting quizzed on my pharm cards. In the first site visit, Dr. Davidson gave me some suggestions about how to identify the most important adverse reaction. For example, even though the hypersensitivity reaction is more serious effect, it is already well-known by most of the people.  I’d better to choose more specific adverse effects when I prepare my cards.  Therefore, I searched each medication by using UpToDate. And I organized the adverse effects by the defined frequency. For example, I firstly mentioned the adverse reaction which usually presents more than 10 percent of the patient, and then it’s the frequency around 1 to 10 percent.

After testing my pharm cards, I presented my H&P. My second case was about a patient with pneumonia sign and symptom. Then I presented an article about how the patient’s BMI related to the risk of the fall. In order to write an effective HPI, Dr. Davidson suggested that I can present my case started with introduce the patient’s current chief complain, and then I need to know what the overall status of the patient is and whether it’s likely that the person will be discharged. And she suggested that when seeing an elderly, it’s important to know their baseline status, such as the person walk with or without device, transfer on their own, and is their cognitive status an issue. My site evaluator also went over the important points for the pneumonia after the presentation of my case, which I found to be very helpful.
