In the mid-rotation evaluation site visit, I present a patient with complains of vaginal bleeding. This is the first patient I interviewed during my GYN call shift. The patient is 37 years old, and she EGA 7wks5d with PMH ectopic pregnancy last year presents to ER complains of vaginal bleeding for 3 days. She has vaginal spotting initially and then became heavier this morning. The pelvic exam shows closed cervical os, blood noted in vaginal vault without signs of blood clot. Types of patients you found challenging in this rotation and what you learned about dealing with them After finishing my whole rotation, I found OB patients to be challenging for me. Their management can be completely different based on their gestational age, such as prenatal care, routine checkup during pregnancy, and also delivery. There are also different tests that needed to be order based on trimester. I found that there are no short cuts to learn this. This is the first patient I interviewed without attending doctor or PAs around, since she is a Chinese speaking patient. The patient is in her mid-30s, and she has hallucination, and she presented with suicidal thought that she reportedly wants to kill herself to join her friend, who died last year. Most of the time, the family members or friends were the first person who noted the patient’s symptoms. Due to the disorganization, the patient also presents poor insight.
